ABSTRACT

OBJECTIVE:To estab lish the evaluation criteria for repeated usage of TCM decoction pieces and Chinese patent medicine, and to provide reference for prescription comment. METHODS :By modified Delphi method ,15 experts of TCM were selected, who had plenty of clinical experience. The questionnaire data and expert opinion were collected and summarized to establish related evaluation index system with two rounds of questionnaire survey. RESULTS :In this two-rounded survey ,experts’ positive coefficients were 100%;authority coefficient were 0.711 and 0.752;reliability of questionaire were 0.830 and 0.926; coordination degree of experts were 0.579(P<0.01)and 0.721(P<0.01),respectively. The evaluation criteria for cross-using and repeated usage of TCM decoction pieces and Chinese patent medicine ,which contained 4 first-level items (component repetition, function and indication repetition ,tumor repetition and others )and 10 second-level items ,were established finally. CONCLUSIONS:Established criteria are with well expert ’s positive coefficient and high authority ,and good reliability of questionnaire. The evaluation criteria for repeated usage of TCM decoction pieces and Chinese patent medicine is formulated preliminarily.

ABSTRACT

OBJECTIVE:To systematically investigate the management mode of non-medical prescribing in Britain and its enlightenment to the establishment and implementation of non-medical prescribing in China. METHODS:By retrieving domestic and foreign literatures,non-medical prescribing in Britain was introduced systematically in terms of development history, types, core elements, implementation status and effect;the suggestions were put forward for the development of non-medical prescribing in China. RESULTS & CONCLUSIONS:Britain implemented non-medical prescribing since 1994. After years of practice and a series of legislation and revision, independent prescribers (including nurses and pharmacists) have owned the statutory right to prescribe any medicine for patients by 2012. In Britain,non-medical prescribing is divided into independent prescribing which mainly exists in community pharmacies and is limited to specific diseases, supplementary prescribing which exists in hospitals or clinic and needs to be signed with doctors. The management institutions are mainly composed of one state administrative department (British Department of Health) and three professional associations (British Nursing and Midwifery Council,The Royal Pharmaceutical Society of Great Britain,The Pharmaceutical Society of Northern Ireland). There are detailed and strict stipulations on the qualification and corresponding responsibilities of the prescribing authority. The British government has provided legal protection for the development of the non-medical prescribing model, and the number of non-medical prescribers is increasing. The implementation of this model has increased the patients'access to medical services. Drawing lessons from the development of non-medical prescribing management in Britain, our country needs to improve legislation, provide legislative protection for the implementation of non-medical prescribing, expand the scope of practice of nurses and pharmacists, establish and improve the training program of non-medical prescribing professionals to promote the development of non-medical prescribing model in China.

ABSTRACT

Objective The objectives of this study were to use Nutrition Risk Screening 2002( NRS2002) to conduct nutri-tional assessment research on patients with advanced cancer in our hospital, and to assess the patients′nutritional deficiencies,nutri-tional risk and nutritional support, and to discuss the nutritional status and clinical indicators of patients with different tumor types in order to provide a scientific evidence for individualized nutritional support. Methods Patients with advanced tumors met the require-ments were enrolled from January 2016 to February 2017. Nutritional questionnaires and anthropometry were conducted and recorded the information of measurements and relevant laboratory tests. NRS2002 was used to screen nutritional risk of patients. Results The nutritional insufficiency rate was 19. 54% in 517 patients with advanced cancer and 49. 52% in nutrition risk. The proportion of nutri-tion-free patients receiving nutritional support was 14. 56% ,and the nutritional support patients with nutritional support were 63. 67%.Theaveragelengthofhospitalstaywas(14.43±11.82)daysforpatientswithnutritionalrisk,and(8.29±6.93)daysforpa-tients without nutritional risk. The incidence of nutritional risk in patients with digestive tract cancer was higher than other tumor types. Conclusion As an effective nutritional screening tool,NRS2002 can help clinicians to screen the potential nutritional risk of patients in oncology and provide the basis for patients to develop rational nutrition support.

ABSTRACT

ObjectiveTo analyze the application of β-lactam antibiotics and bacterial drug resistance in our hospital for providing basis of the rational use of antibiotics.MethodsThe DDDs,total consumption sum,DDC and bacterial drug resistance of β-lactam antibiotics during 2008-2009 were analyzed statistically.ResultsCephem Antibiotics was the most extensive application, and the DDC of Benzylpenicillin Sodium was the minimum in the β-lactam antibiotics. There had bacterial drug resistance of most β-lactam antibiotics, but the differences were not significant.ConclusionThere was irrational use of β -lactam antibiotics, and it was important to enhance antibiotics supervisal and drug sensitivity before use for decreasing the abuse of antibiotics.
